Application form for document verification


  • Download online Challan for your payment purpose by clicking onOnline Challan.
  • Fill the Application form electronically/online carefully.
  • NOTE: Your application form will be processed after your fee Verification. Therefore, bank Challan number shall be written correctly.
  • Hard copy of the Application Form and documents should not be sent to the Federal Board as online applications serve the purpose. However, if the applicant has passed his/her examination on or before 1989, he/she should send scanned copie(s) of relevant documents at the E-Mail:, Or hard copie(s) may be sent to the following mailing address along with application tracking ID:
                  Deputy Secretary (Certificates)
                  Federal Board of Intermediate and secondary education
                  H-8/4 Islamabad.
  • The verification fee for each document is Rs.330 for inland student and US$ 20 for overseas students. If you have paid the fee for one document and checked/ requested more than one document , the board will issue the document of latest Examination.
  • Board will not be responsible for misplacement of the verified document if the postal address given below is incorrect.
